This service will provide necessary support in order for the member to remain in his or her own home and/or participate in work or community activities and maintain safe and sanitary living conditions and maintain personal cleanliness and activities of daily living. The objectives of this service are to help Division members develop knowledge and skills, assist in learning socialization and appropriate behavior, and support them in achieving and maintaining a high quality of life. Essentially, it empowers individuals to become self-reliant.
HSK services provide a safe and sanitary living environment for the individuals in our care. We uphold the cleanliness and organization of both personal and shared spaces within their homes. This is accomplished through tasks such as vacuuming, cleaning bathrooms, changing and making beds, doing laundry, shopping for groceries and household supplies, among other duties.
The goal of the service is to provide a “break” for the caregiver. This service can be provided in the member’s home or in the community. In carrying this service, the agency will ensure the following duties are done:
Transportation to employment, day treatment and training, medical appointments, visits with family and friends, when necessary, school, and other community activities shall be the responsibility of the Developmental Home Provider. Other reasonable transportation within the community is also the responsibility of the Developmental Home Provider, including fees associated with the transportation.
